First of all Adder, I'm a guy so I'll give you advice from a guy's perspective but, also listen to the answers from some of the ladies on here...
they can give you valuable insight from their point of view.
Here's what I finally did to muster up the nerve the first time I wanted to ask this cute girl to dance with me.
Let's say you and she are at the dance or whatever...you want to ask her to dance but, something is holding you back and talking you out of it....that's your nerves...you're nervous simply for this reason: you worry that she might say "no".
In my case I simply took a deep breath, and had a little talk with my self..."self, I said...this is the opportunity you don't want to let go by without at least giving it a chance...so, let's just go do this"
Walk over to her, look her in the eye, smile politely and ask..."would you join me for this dance?"...or, be creative...but, most of all...be yourself, say whatever you're most comfortable saying....
bottom line...just do it!
don't let the chance go by without giving it a try...the worst that can happen, she says "no thanks, I'd rather not."
The best thing, and what probably will happen is she will say with that cute smile on her face, "sure, let's go!"
From that moment on, you will have conquered one of the biggest hurdles a young man has to face...the fear of rejection...and you will have the confidence you need from then on to never hold back from asking a cutie if she'd like to dance.
Good luck!

Consider that the girl may be feeling the same things. Learn to be more confident by realizing that she (usually) wants you to ask her to dance. Stop worrying about how you look or what she is thinking, this attitude radiates self assurance and is a good thing. Also, remember nerves are natural, learn to embrace your weaknesses to become a stronger person. ♥good luck♥

Fake confidence. Ask yourself before you go up to the person, "What's the worst that could happen?" She'd say "no, thanks." What then? It's not that big a deal really.
