The EHIC replaced the E111 and is free. You can apply online and you will receive it in a week.

It's obsolete anyway. I only found this out tonight as it was a question on University Challenge. The E111 was replaced by the EHIC, European Health Insurance Card in January 2006. It is issued free of charge. http://www.dh.gov.uk/PolicyAndGuidance/HealthAdviceForTravellers/AboutThisSection/fs/en?CONTENT_ID=4135688&chk=VQx%2BrT

No - E111 should be obtainable by filling a form at the Post Office

No just apply for a new one and if you are going on holiday before it comes back just call and they will give you a reference number to quote if anything happens and you need to seek medical advice.
