Well, if I am not sure then I dont apologize initially but if I have doubts that I hurt someone then I ask them straight and clear the pic & if I am wrong then I dont hesitate to apologize.
But definitely I dont ask for apology left-right. It will lose its mening & wont appear heart-felt .

as a guy who likes to test barriers in conversation...and test people's stances on various issues that might be very opinionated..like gay marriage or iraq....i always give an introduction to certain things i am going to say...like, " i dont mena to offend anyone" or " i know you might be a liberal, but..."
