When your playing truth or dare, if someone decides not to do the truth or dare, what would be a good "punishment" besides kicking them out of the game or locking them in a closet!?
maybe like eating something nasty..but wat??

Get out the blender and blend together a whole bunch of things that you all, including them, absolutely hate to eat. Make them eat the disgusting "smoothie."

if they refuse the dare then they must answer a truth question or vice versa
if they still don't do it then threaten to call their crush or some boy (have phone nearby)
then they will either do the truth or dare or you call the boy and say something about the girl or whatever.
-or-
for every truth or dare they refuse they have to remove an article of clothing.

have them do all of the dares that have been done in the game already, including the ones that they already did. If it's really early in the game, so there hasn't been that many dares, you're right, make them eat something gross. Try mixing a little bit of all of the contents of your fridge (unless it's something that will make them sick or something that they can't eat) and make them eat it.

hot dog with custard. Make them wear a pair of pants over their clothes and send them outside for a few minutes. Put their hair in bunches, take photos...or send them round the block, especially nice for the guys.

make them run down the street naked shouting "I pooped my pants"! or they can do it with clothes on if necessary.
make them moon the outdoors via the window for 5 minutes( preferably one facing the street)
make them drink vinegar
ice in the underwear until it melts
