Yes she really did. She went into a salon. And they told her to think it over and come back. But she shaved it herself. at people.com you can find photos.
I thnk maybe it was another attempt to get attention. Or rebelling to show that she is tough. Or a reaction from her divorce and stress,

Oh, yeah. She's as bald as a brass rail now. While I'm not sure why she did it, I assume it's because she finally realized she's losing popularity and fans and this is a desperate attempt at making sure she stays in the headlines.
