If pregnancy occurs, it is important to remove the IUD immediately (that day). The normal spontaneous miscarriage rate is about 18-20%. For women who conceive despite an IUD, the miscarriage rate is about 25% when the IUD is removed immediately. If the IUD is left in place, the miscarriage rate increases to about 50%, and many of those are septic mid-trimester losses which are particularly unpleasant and which are associated with subsequent infertility in some cases.

Having the IUD can wait another two days, it's already been in two months.

Yes having an IUD increases the risks of miscarriage but only if it isn't removed in the first trimester, but that's it and really two days isn't going to make a difference.

Although the pregnancy rate during IUD use is very low (less than 1% per year), it is not a 100% effective method of birth control. If pregnancy does occur, presence of the IUD increases the risk of miscarriage, particularly during the second trimester. It also increases the risk of premature delivery. These increased risks end if the IUD is removed after pregnancy is discovered. Although the Dalkon Shield IUD was associated with septic abortions (infections associated with miscarriage), other brands of IUD are not. IUDs are also not associated with birth defects or other pregnancy complications.[26]

"What is the appropriate management of a patient using an intrauterine device who gets pregnant?" This question from SPIRO POLYHRONOPOULOS, MD, of Vancouver, B.C.
If the patient using an IUD is pregnant there is a 2 to 3% chance that it may be ectopic. Therefore, careful evaluation is necessary. If it is an interuterine pregnancy and the patient elects to continue the pregnancy, the IUD should be removed as soon as possible if the string is seen. If the IUD is removed in the first trimester, the risk of second- or third-trimester fetal loss is not increased. If the IUD is left in, the woman should be advised that there is an increased risk of abortion in the first trimester, of septic abortion abortion especially in the second trimester, and of premature birth in the third trimester. If the string is not seen, the IUD can be removed under ultrasound guidance if it is located inferiorly to the gestational sac. If located superiorly, the IUD cannot be removed without an abortion. Unless you come up with a medical degree and a specialty in obstetrics, I would take the advice of those that already have one and have her wait till Wednesday. If you're really concerned, call the local hospital -most hospitals have nurses available to field these kinds of calls - it's like getting a second opinion.
