Are you in the US? Are you a citizen or in the US legally? If so, google the name of your state and the following: "name change" adult

You will probably be able to find examples of the forms you need to submit for the name change. Fill out and print these forms and take them to the clerk's office in your local courthouse. Fees will probably be about $30-100, plus you will probably have to buy space in the classified section of the paper with some sort of statement about the name change.

Within about 30 days, you should legally have another name.

If you are in another country, or not in the US legally, sorry, I don't have any advice for you.

Go to the court house in the county you live in and fill out a change of name request form if approved it costs about 100 dollars

I cant type well enough long enough but the I.N.S. can tell you all you need to know just hope you were clear back home does head in noose mean anything.I have V.N. buddy named douglas he said it was pretty simple
