The sound seems to be coming from outside the engine. I thought maybe it was a common problem with these engines with over 100k miles my friends Astro does the exact same thing and its a 4.3L

Maybe it's just the valves "ticking" until Oil gets up into them, or maybe there is something in the fans way and makes contact until it blows it away from it...

Other than that, i don't know what to think. one day get under and over the area and check to see.

The idea of using a long screw drive is a very good idea to help locate the noise. However you should also put the tip of the screw driver on your timing cover, behined the belts. Some times when the chain is worn it will hit the cover and sound just like a lifter. Test twice and replace once, after all parts are not cheap. I've also had very good luck with running a heavier oil if the lifters are the problem. Good Luck!

Find the longest plastic screwdriver you have and place the plastic end on your ear opening. With the motor running while making the ticking noise place the bladed end of the screwdriver on the valve cover one side at a time determine weather the noise gets louder. If so you have a hydraulic lifter sticking. Change your oil and filter add a quart of Rislone and fill the crankcase with Havoline, Valvoline, Quaker State, Mobil 1, Brad Penn or Gulf motor oil. Drive your truck for a month. If that doesn't fix it replace the offending lifter.

It could be the lifters or the rocker arms making noise because the oil has not pumped up the lifters. Does that truck have a belt driven fan or an electric fan? The fan clutch should only make one tick sound when the clutch engages that's it.

this could be a rod bearing.does it need an oil change? the front rod bearing will rattle if the oil is too thin or the clearance is off. if it goes away after its running awhile change the oil with a little thicker oil like 10w30 instead of 5w30 that they recomend and hopefully prob is gone if not you can live with it until knocks all the time or put new bearings in it
