In telepathy, when a person communicates telepathically with the world, and when people are talking to that person...

Does the world hear the voices of the people that are speaking to that person? Or does the world only hear the person's thoughts about what the person's saying?

When a telepathic communicates telepathically, they can control they powers so that only the person they communicating to will hear it and they can communicate their thoughts and voice, but they can not communicate the voice of others. They can also read other people minds and move objects with their minds.
