Is it determined by the crotch area, if it's saggy and/or full of holes?
or is it by premanate stains (no matter how clean they are)

There are no limit on that... if you feel comfy in it.. then it is fine

It all depends on what you are using them for! LOL And also it all depends on how much you can spend for a new pair let's say once or more a year!. It all depends also on how often you wash them and if you use bleach or not, fabric softener or not, etc, etc, etc...If it is the crutch area you are worrying about, you can always sew a patch where the holes are ! That'll give a new life into them!
