In WW I Italy fought beside the Allies, participating decidedly in the defeat of the Austro Hungarian empire.

In WW II Italy fought beside the Axis . Germany Japan, Bulgaria, Hungary, Finland and others. But its participation was very poor.

All of Italy´s interventions were a disaster in Greece, Africa.
More than once Germany had to urgently help its partner so disaster would not befall Italy.
Italy Finally surrendered in 1943.

In World War II, Italy joined the Allies only after the Allies pushed the Italians out of North Africa and the Italians deposed Mussolini. The biggest difference, in my opinion, would be no "Big Four" at Versailles, as the fourth was the Italian prime minister, Vittorio Orlando.
