This green hag/witch is a continual cause of death and destruction, in a sort of wonderland. There may be others of the same appearance also. It isn;t The Sword in the Stone, but similair. She seems to come alive when you open this bok.
She is eventually killed when someone slams shut the book, with her face between the pages. Inmanaging to get it shut, she is "stamped out."

is it the carebears? I remeber a movie with the carebears in it with a book that tries to destroy the world after a boy opens it up. Then he had to shut the book and lock it to stop it
