No. I know what's internal and external. The person most prone to losing themselves in deep thought (if they're sane) would still easily distinguish the inner and outer world.

A man in contemplation of his inner thoughts realizes that his mind is made of a dual thinking of either good or reasoning which stem from logical principle. The reality itself is beyond all good and evil but is congruent with a person's individual life, which is always a battle between the choices of good and evil. The individual itself is the only being that chooses which act to follow and believe in, and the hope of all hopes wishes him to choose goodness over evil.

Perhaps this would better be asked in the psychology category?

Our perceptions of the outer world, if skewed in our mind, would appear as real and we wouldn't know that we're unable to distinguish from reality. We could claim to see reality as it is, but would not know of distorted lense from which we look.

My thoughts do not conform to the world frequently, because I really believe that there is good in all people, and I have the emotional scars to prove it.
