if your in the philippines, just go to the police station and ask them to make a report for you and ofcourse you have to give them the details of your report. but i'm not that sure if it will cost you some peso.

What was the crime and where did it happen. Give details. If it happened in the Philippines, you should be here in the Philippines to file such a police report and in the location where the crime happened. If a Filipino national committed a crime against you in your locality or area, seek assistance from your local Police. They would probably refer it to the Philippine Police authorities.
Try checking the National Bureau of Investigation (NBI) website. They are the equivalent of the FBI in the US. However it is strange that their website is not updated. In fact the NBI Director featured in the website has died a year ago and the Justice Secretary has long resigned.

If a person has been arrested, the information is public and you ask for a copy of the police report. If what you want is an investigation by the police of someone, you will have to file formal charges, If you just want information about someone, this is private and will not be given to you by the police, even if they have been investigating.
