If you do it like that it's like writing on a letter;
England, Great Britain.
or
America, United states of America.
The Netherlands is the official name, Holland is just a small part of the Netherlands.

The name of the country is the Netherlands. Two of the twelve provinces are North and South Holland in the area of Amsterdam and Rotterdam.
Many people in the Netherlands use and accept Holland as a nickname but it is not truly correct.

No. "The Netherlands" should be used as the address. Holland is not the official name of the country.

Not really. Most clear is The Netherlands. You can add the appropriate province, but it's not really needed.
You might just as well put Holland on it. Officially this stands for a part of the Netherlands, but really, they'll understand. (As long as you add the town and street of course)
If you put both..you might confuse the poor people.
