go see you GP tell them what you feel you are
they will send you a counselor or psychologist
and a gender doctor /endocrinologist (hormone specials)
you will need to see all of them over a bit of time while getting blood test taking hormones getting counseling etc
you then transition around the same time where you change your name with licenses, banks, health cards, tax and other id's etc
over to your new name you then start the R.L.T. (real life test)
go see a surgeon and prepare for the last stage the operation

but before you do any thing read up as much as you can

it all takes time and is not very easy on the emotions your body
but if you really want to become the person you really are it is all worth it

That's a tall order, but I'll give you the brief procedure:

Male-to-female:

1) Gender counseling
2) Hormone prescription (estrogen)
3) Possible breast augmentation (optional)
4) Genital surgery, done all at once, or in parts

Female-to-male:

1) Gender counseling
2) Hormone prescription (tesosterone)
3) Masectomy (removal of breasts)
4) Genital surgery, either metoidioplasty or phalloplasty

Those are the basic ones, which you can look up on your own later. And remember, some transsexuals choose to be non-operative, meaning that they never actually get genital surgery, for a variety of reasons (usually the very high cost).

Get diagnosed by a doctor as being a candidate for sexual reassignment surgery. You will then spend several months living the lifestyle of the sex you are intending to become and going counseling. After that is done, you will be given hormone treatments to take in anticpation of the surgery. After a few months of that you will then go under the knife for a series of surgeries that will reconstruct your genitalia, remove or add tissue as needed and made you into the opposite sex.

Several years. First you have to see a therapist for a time for them to determine if you are mentally ready and stable eough to make the change. Then they start you on hormone treatements that last about two years, and then comes the big surgery to make the final changes.
