Usually your teacher will give you an example (or several) and tell you what they expect. Follow these guidelines.

Apart from that, it is good to start out with your name, student number, the date, the class, the teacher's name, and any other basic info that might be relevant. Then provide a title and a one paragraph summary of the results (usually you would insert this after completing the body of the report).

Since it is science, a good outline would include a hypothesis, a discussion of variables and possible confounding variables, and some background to show that you have done your research. The hypothesis should be as conservative as possible (e.g., "the patient is alive" not "the patient is dead").

Then you would describe the results in greater detail, including any surprising results, and conclude with respect to the hypothesis (did the results support it or not?). Make this part as clear and as graphic as possible.

It is useful to follow up with a discussion of the significance of the result(s) and possible future research directions and/or experiments that might logically ensue. The latter is especially important if confounding factors were discovered in the course of the experiement and you have some ideas about how to eliminate them.

Finally, be sure to include a list of references to acknowledge that you are "standing on the shoulders of giants" and so that an interested reader can compare your readings to their own and let you know if you've missed anything significant (or upgrade their own research).

I used it for my report. make sure to put data in (visual when possibe) graphs, table and if you have any picture put them in the back before refs and title them (figure 1, table 2 graph 3) short description exampel: metabolic rate of protein for a 10 year old is compared to national avg ect.. refrence to them in the report i.e as it can seen in the figure 1
