There have been a few bad guys that have gotten away. One is the little sister who took the wrap for her brother, then there is the one where the special effects guy was killing men with the same birthday as his father I think. And finally there is the killer who leaves the models of the crime scene.
I think that is it.

I don't know about Las Vegas. But if I remember correctly the bad guy got away twice in CSI: Miami!

Yes. There was one where a popular high school girl was murdered. In the first few minutes of the show the defendant's sister took the stand and confessed in a blood stained shirt.

Long story short, the girl was 11 year old genius and claimed she killed the girl. Gave Sarah mad guilt. The lab doubted, and the evidence could go both ways. In the end the boy was found innocent with the lab going to pursue the younger sis. She told Sarah that she lied and her bro did it.

The bad guys have got away a few times now but I'm sure Grissom and the team will catch them eventually.
