That's an English idiom for a person's favorite friend. It can mean a romantic relationship but it doesn't have to be.
It can have all sorts of meanings, and it doesn't have to be a human person. Some people say God is their best friend.
Also there's a saying that a dog is a man's best friend, becuase your dog will love you even if all humans hate you.
