People who are confined to mental institutions are considered insane. Those who are not so confined, are considered sane. There is no other definition.
Subjectively, all who do not see things in the same light as the observer may appear to be insane.
Added;
Dave S below has a good description of one manifestation generally accepted as indicative of insanity.

well.. insanity has a lot of different levels..
1) depression..
2) hullicinations (you know what i mean right?? lol)
3) feeling of alone...
4) loss of a close friend or family member
that is what i would suggest. if you really feel that you are "insane" then you should go to a therapist or a psychologist.. no one can help you better then they can and thwy can tell you exactly what is wrong..

what is insane? people all around us look at others and say look at that person and they don't understand them we all walk different paths and what is insane to one person might be ok to others we are all different so who really has the right to say who is insane or not where i draw the line is when one person goes out to hurt another

Insane is only a legal term for one who is not able to help in his or her own defense. It is not a scientific term for a mental disorder. There are no longer any such things as "Insane Asylums."

the prinicple that constitutes insanity is a thought rationalization that differs from society's, people who are labeled insane don't think they're insane, they consider their actions, it is society who labels them with degrading terms based on their ideology of rationality
