With a smile and a firm (but not crushing) hand shake while saying "Hello, I am......." They will probably ask you to sit down. Take a seat and get comfortable without slouching. Don't cross your legs as that looks too relaxed, keep both feet flat on the floor.
Your style of dress is important also. If you are interviewing for an office job, wear a suit and tie. If it is another style of job, wear the type of clothing you would wear on the job, just make sure it is clean and pressed. Show them that you thought the interview was important enough to you so your got fully prepared. Take the time to shine your shoes, too. A bad looking pair of shoes can make a great suit look shabby.
Remember this too; you are being hired to fill a need for that company or organization. Do some research and find out the challenges the employer is facing. You may not have the answers, but even knowing the challenges faced by the supervisor can put you up on the employment competition.

You should dress smartly for the interview and should leave home earlier than you need to on the day of the interview - you may be delayed by traffic or for other reasons. Be courteous to all employees of the company. At the interview itself you must be positive about yourself and your abilities - but do not waffle.

Tough question. You should play off the interviewers wants - it's his time - but the time he gives you to speak - you speak well.
It's not crazy to reherse statements in your mind. Look up some expected questions and answer them already in your mind. Obviously, don't sound too rehearsed.
The typical ones are
1. best thing about you
2. worst thing about you
3. best thing about your current job
4. worst thing about your current job
5. what type of management you like the best
they'll be some questions typical for your field, so answer them in your mind also.
I know it's not fair, but that 20-45 minutes will dictate a yes or no. Everyone goes through it though.
