someone who you know for sure is nuts... something like "hello, you're nuts"? I don't know how to react in such a situation... do you?

Were you the one being told you are nuts or someone you know is nuts? If it's the latter than you have to seek someone in the Mental Health profession to assist. I had a roommate who "bugged out", Apparently had mental breakdowns as a youth. A neighbour whom worked in a mental institution oddly enough was able to determine he was showing signs of manic depression. I then took him to his family home wherein his family put him in the "nervous" hospital. The End.

That reminds me of when me and a girlfriend of mine were at the drive thru window and there was this big commotion that was going on in the truck behind us and out jumps this girl and she was cursing and yelling and then she walked by the car and we just looked and she got right at the front windshield and said "What the "F" are you looking at?" then she walked in the fast food place. My friend never took crap off of anybody and we just stared at each other for a minute and she said "I would've got out of this car and beat her @** if I hadn't been so shocked by what she just did" and we both started laughing. I guess they got their order wrong! LoL!

I don't recommend it. I did it once- he wasn't nuts but he was odd and was always singing. The result wasn't pretty. I can't remember what he answered but I felt like a fool and it bothered me so much I later apologized to him.

Depends. If they say something totally wacky or tell you about a situation that is way out there, I might say something like, "you might consider counselling". If they are acting weird, I may say, "You know, they have medications to help that"
but out of context I dont think I could say it

Sure, I'd just say "We should go for lunch in quiet places with neutral settings because you're nuts."

Yeah, I did. Wasn't a whole lot of fun. It was my mother. Now she's on meds and everybody is much happier.
