Dementia-community education program for the reduction of alcohol abuse, also think stroke or anything else that will reduce injury to the brain

Depression - clinic screenings for depression and education related to depression and treatments available, reduction of negative image of depression, out reach for groups with high relationship to acute situational depression (divorce, grief groups, caregiver support ect)

prostate...PSA screening? (I just do the TURPs)

brain injury, bicycle helmet education and laws, research which areas have increased incid. of injuries and educate in schools of that area, seat belt education, motorcycle helmet education, parent education for sign/symp of first onset trauma so they know when to seek help, first responder education to know to not move injury victims unless threat

epilepsy-education prenatal care to reduce premies, reduction of infant hypoxia related to infant position/choking/suffication on blankets/ reduction of childhood posioning, reduction ETOH/drug abuse

copd is of course smoking and industrial enviorment use of masks

asthma again adults smoking around children

If you figure out the what/why you need to teach to the community (and it will mostly be group teaching unless it is screenings) and who you need to teach (age group most effected at onset or age group most overlooked but in need as in depression and the elderly) the how, when and where should be a simple creative exercise. (as in if you get to do that part) Good luck
