arcadia has got to be right the only things you missed in your question is they have won all four divisions and won the league cup along with the fa cup.they also won the uefa cup back in the days it was known as the fairs cup or texico cup.your answer is wolves.
burley have played in all divisions won the league and fa cup but i dont think they have won all divisions.

Nicky Butt is one (relegated in his loan season with Birmingham). Sheringham would arguably qualify if he was still a premiership player as he was relegated from the old first division with Millwall. Dunno the others, would be interested to know. Added: Good spot on Kanu chaps, however, Dwight Yorke was with Sydney FC in Birmingham's relegation season, so it can;t be him....
