The whole of the tooth fairys house (castle) was made of childrens teeth she had collected and the book had a blueish cover.
I had it when I was little but can't find it anywhere. I'm 23 so this was probably about 15years ago.

What Do the Fairies Do With All Those Teeth was published in 1996, so may be too new to be your book. It does have a blue cover, however.
