This term is a colloquialism which means it is not to be taken literally as a heart cannot be on a string, of course. Let's say that you are in love with someone but that someone either does not love you back or doesn't tell you whether or not he loves you. So he keeps your heart on a string waiting for him to declare his love. When your heart is on a string, you are being kept dangling because you do not know whether someone loves you or not.

It means that someone is toying with you. Invision a kitten. Dangle a string before it. The kitten will pursue the string and torment it when caught unless it looses interest first. I wouldn't want my heart attached to that string. Nor, do I wish to be a puppet on a string (Elvis Presley song).

I think it refers to your "heartstrings". There are phrases such as "tugging on your heartstrings", which means that you feel sympathy for something.

Heart on a string might also mean "heart on the line", meaning your heart is at risk of being hurt, etc.
