I've had the displeasure of both in the past. Been to some good, and I quote Viviana, "Mental Hotels". Food, bed and both people on your level and worse. The ones that are worse just show you you're not that bad off after all.
Was in one in Ohio and another in Salt Lake that had under ground tunnels. You wouldn't believe what went on down there.

Institution, because I live in a very cold and snowy place with temperatures in the negative digits right now. It would be warm and dry and you would get food.
