yeah, it happens all the time... besides, i don't think they'd do it on purpose.
the best thing to do is to just keep going.. i was in a band and one of the members would get so flustered that he'd just stop. mistakes aren't really noticable unless you point them out.
Kirk Hammit says something like, if you mess up, just play it 4 times just like you did during the mistake and it will sound fine.

If you go to a lot of live shows, you're almost sure to see a musician mess up every once in a while. Although, since they are mostly playing their songs, that they play all the time, it is rare to hear pros mess up.
But yes I have heard it. In fact there is a big mistake that Billie Joe Armstrong from Green Day makes on the live CD Bullet in a Bible. He hits a wrong chord in one song(one of the American Idiot songs I think). That show was a very large show in England.

Oh yeah, but I am a classical music fanatic so I haven't heard it happen that often because I would listen to recordings most of the time and recordings are done by you know, World Class orchestras but there are instances.

Yeah, I have have! I have perfect pitch so I can hear if they make a mistake very well. Even when I was at a Panic! At the Disco concert they hit the wrong cord!
