Can I trade or exercise them in the after hours or do I have to do it by 4 p.m.?

There is no after hours market for options so all trades must be done by 4:00 Eastern time. (Note: I am assuming you are talking about stock options. I believe options on some indexes, futures, and commodities have different trading hours.)

The OCC accepts exercise notices after 4:00, I believe until 4:30, however your broker will have a cut-off time which is probably earlier than that. You will have to check with your broker to determine the latest time that you can submit a request to exercise options you hold.
