I don't know what you would do, but I would not be able to hear the blast , cuz the only thing I would see is an extremely bright light in milliseconds timeframe due to proximity of defense plants , oil storages, and nuke power stations...... First immediate thought would be "OH SHI*"

Since the demise of the former U.S.S.R. there really are no nations that can decimate the U.S. So, with that considered, if a nuclear war were to break out it is likely that our opponent wouldn't be capable of launcing numerous nuclear weapons at the U.S.
It would be most likely that our enemy would launch first. Against any current nation we have any sort of conflict with, the U.S. has superior miliary strenght and should not have to resort to nuclear weapons except in retaliation for a like attack.
Having a limited number of nuclear weapons, the enemy would want to select targets strategically. Targets would likely be selected based on (a) population density in order to inflict the greatest number of casualties (b) symbolism such as attacking the nation's capital or (c) strategic in order to reduce our ability to retaliate. The last selection would only be of value to a larger opponent like the former U.S.S.R.
So, most likely, if you didn't live near a major city, you could avoid radiation by simply being far enough away. The only concern would be if one lived in the path of the fallout. Those people would have to move or take shelter. Most would be physically safe although most would be psycologically scarred from such an attack. Life, however, would go on. Japan is far smaller than the U.S. and survived not one, but two atomic bombs and has become one of the more prosperous nations on earth. It is survivable.
Whether we responded in kind, would depend on who are enemies neighbor were and the likelihood of fall out from a nuclear blast imapacting them. If it could be done and only impact the enemy, it would be done. If we are allied (or just don't have a problem) with the attacker's neighbors a conventional war would begin (or continue).
More likely than a current adversary attacking the U.S. is that it would would attack one of our allies, Isreal. Many nations in the middle east don't exactly care for the Isrealis and they are easier to attack with a missle than we are.

Panic, probably. I don't think the governments around the world are at all prepared for this, no matter what they say. Though I am pretty confident that there are hugely safe shelters, and procedures in place to protect CERTAIN people. i have a horrible sinking feeling that the rest of us will be pretty expendable in the grand scheme of things. In short, I think we will be either lucky, and survive, or we won't.
