Starting from least rare to most:
Rare, Super Rare, Ultra Rare, Ultimate Rare.
Rare - Silver writing for the name of the card.
Super Rare - Holofoil effect used for the card picture, no effect used on the card name.
Ultra Rare - Holofoil effect used for the card picture, gold letters used to write the name of the card.
Ultimate Rare - The card is placed in a special holofoil effect, that looks different from the typical holofoil effect, plus other sections of the card that would not contain holofoil normally (such as the Japanese symbol used to represent the card type/attribute, as well as the Level of the monster (if applicable), do show up with the same effect. Ultimate Rare is a variation of cards that can be usually Rare, Super Rare and Ultra Rare.
There are other rarities out there (Secret/Promo, Parallel Rare etc), but I'm just sticking to your question.

The rarest? well, there are some collector cards that cost thousands, like Des Volstgalph. But only certain versions of those cards are actually worth a lot. For a kid's birthday, just get a booster pack or something like that. If he plays yugioh, get him a structure deck. god cards are usually fake, and exodia pieces are pretty much useless for most players. If he likes collecting, any of the cards mentioned by previous people would do.
