Tell them (if you want to) what you are doing right then, how you are feeling, what you plan on doing later or something you just found out. Stuff like that. What's up, wassup or what r u up to means, "What's happening with you right now?" With friends and family you can be more specific and personal if you want to when you answer. Strangers say that to people almost the same way they would say hello. Usually with people I don't know or will probably never know I respond with something like, "Not much". Whatever, just something short but polite. Hope this helped you.

Usually it's just expected of you that you repeat the phrase back. It's lost it's meaning in modern use and is basicly an empty greeting.
If you're creative enough to make this sound good, sometimes it's fun to throw off the person you're talking to by naming something that is literally "up" in any sense of the word.

Reply with "Wassssup, yeah".
