i only remember parts of this movie but this man is falsely accused of killing his wife and is being relently pursued for the crime. A friend who is also a doctor (i think) believes the man's wife was killed by a horse because her body was found in a water trough.
anyway when the man is actually cornered(in a cave?) he has completely lost his mind from the pursuit. any help would be appreciated. thank you

I can tell you that it is not the fugitive. Only because you make mention of a horse trough. There were no horses in the fugitive.

That sounds like The Fugitive (1994) remake. The one starring Harrison Ford and Tommy Lee Jones. Ford plays a doctor who is accused of killing his wife and becomes a fugitive from justice. Your mention of the scene in the cave sounds like the one in the movie. The marshall (Jones) corners the doctor in a drain pipe overlooking a river. The doctor says "I didn't kill my wife" and the marshall says "I don't care". Classic.
