I am betting you have a bamboo plant. They are commonly referred to as "Money Plants", "Good Luck Plants" and "Fortune Plants".

This same exact thing happened to me when I transplanted my bamboo from one vase to another. First the leaves and sprouts turned yellow. I did an Internet search and it instructed me to cut off the infected area, but I was too late. The rot went into the stalks and the plant couldn't be saved.

If the stalks are turning yellow, I fear that it may be too late for your plant as well.

It appears other people agree with me. The author of the article "Lucky Bamboo House Plant" states "If one of the stalks turns yellow it is beyond help. Remove it from the container or it may sicken the remaining stalks".

Here's a link to Wikipedia's site about the Jade plant, also known as the money tree. I'm guessing this is what you have. Sounds like overwatering, but check out the info on the site.

Of course there are several plants referred to as "good fortune plants." Bamboo, and bamboo-like dracena, and a tree (Pachira aquatica) all share the same common name.

Did you mix any thing up in the soil? If so it is starting to rot under the soil and is getting hot and building up acid around the roots, take it back out and use nothing but good clean top soil. The nice black kind you can smell.
