How much does a starter cost?
I'd need to know the year, make, and model of the car you're looking for. There are two options when it comes to starters--brand new and re-built. New ones are expensive--to the tune of $400-$800--but re-built ones tend to be more reasonable, at anywhere from $150 to $450. Installation will depend largely on the car, but it takes about 1-1/2 to 3 hours on most cars. Some are easier, and some are much harder.
Can a car run without one?
No. The starter is the motor that spins the flywheel of the car to get the engine turning. Without a starter, you can't get compression enough to start the car.
Can a car run with an old one?
If the starter is old but in good working order, yes. You may need to replace the starter cable (from the battery to the starter solenoid), but most are usable. If it's too old or broken, though, then the answer is no.
Your local NAPA auto parts or Auto Zone should be able to help you, and recommend a good shop to do the installation (or do it for you).
EDIT: Dodge Man is right; your problem may not be the starter at all. Be sure you carefully checked the battery, the grounds, and all the connections to the starter. Your problem could be something else entirely--even an ignition switch. It's impossible to tell without specifics, though.

The cost depends on the car. You can go to a parts store like AutoZone and Napa and ask about the price (part only). You can also go to www.autozone.com and www.napaonline.com to find the part and prices after entering the year, make, model, and engine configuration of your car. There's usually a core credit, which mean you get credit when you bring in your old part. You'd have to get an estimate from a repair shop to get an idea about parts and labor costs.
No you cannot start a car with auto trans without a starter. You can find old starters at junkyards, but they might ask you to take it out of their car yourself. I wouldn't install an old starter. You can find rebuilt starters at some shops. These are warranteed.

The cost depends on what kind and where, so: write down the year, and make/model of your car. Is it a 4, 6, or 8 cylinder, automatic or standard. Now: call each of the Automovile Parts stores and get the prices. Two of the larger auto parts stores are: Advance Auto Parts and Auto Zone.
You can buy one at a junk yard, I believe they will give you a 30 day warranty, but for just a few $ more at the auto parts store, you can probably get a lifetime warranty.

How much depends on what kind of car you have. No, you can't run the car without one. And yes, you can get used ones, or you can get what is called remanufactured ones, either of which is less expensive than a sparkly new one.
If you don't plan to keep the car much longer, get a used one. If you are not sure how long you plan to keep the car, get a remanufactured one. If your car is brand new, get a brand new one.

A manual transmission car can run without one, as long as you park it on steep downhills.
Or the '50s G.M. Fluid drives.

You can run without one if you cut a hole at the bottom and use your legs.
