1. A because "teacher" and "principal" are both singular nouns and only one of them is acting, so the verb is singular. People sometimes mistakenly use the plural because there are two subjects to the sentence. But since it is "teacher OR principal," only one of them is acting. If it were "teacher AND principal" you would use the plural form: "Teacher AND principal CONDUCT a safety check."
2. A because "had" clues you to the fact that this is a pluperfect construction: "had torn." "have torn" is perfect. "Tore" is simple past tense. "teared" and "torned" are never correct.
3. D because this is pluperfect: "had swum." Perfect is "have swum." Past is "swam." "Swimmed" and "swammed" are never correct.

1. A and by the way you've misspelled principal
A is the correct answer because when "or" is the conjunction, the verb matches the nearest subject in number. Hence, principal is singular and so is conducts.
2.A. torn This is the past participle which uses the "had" helping verb.
3. D. Same reason as #2

2. A - past tense verb .. happened in past tense
3. D - past tense verb .. happened in past tense
