Have you recently installed any new sound software / sound drivers?
My advise run a quick system restore to a few days back.

Assuming you have windows XP, you should find this file:

C:\Windows\system32\sndvol32.exe
(I've previewed my answer and the paths are getting changed by Yahoo)!

That should be sndvol32.exe in Windows\System32

Right click it and make a shortcut.

Drag the shortcut to the place on the start menu you are used to having it in. It might take a few goes as Windows stores the Start menu in several pieces, but if you continue, you will get it there eventually.

Once you do, you should right click on the shortcut and left click on "Properties".

Click on the "Shortcut" tab on the top of the window and change what is highlighted in the "Target" box to:

%SystemRoot%\system32\sndvol32.exe

That is change the "C:\Windows" part to "%SystemRoot%"

Then click on "Apply" and "OK" and you should have a shortcut that is identical to the original.

You might also want to check the Recycle Bin on the desktop, incase the shortcuts were accidentally deleted? If so, you can just restore them (Click "files" then "Restore").

You can also drag and drop things around in the start menu, so they might just have got dragged into a nearby folder on the start menu?

I hope this help! & Good luck!
