With all these questions - I'd say, you are too concentrated on pleasing the boy. Don't worry, he'll have fun if you do.
1. It is not possible to do it if your feet are completely together.
2. I never saw that, but what you describe sounds kind of bad, on a cheap side. Yeah, it would look totally stupid if the boy doesn't respond to this, and you have no way of knowing if he will or not.
3. You have no way of knowing what he wants. But he'll give you some hints. You can also give him hints as of what you want better.
4. Same way, but your feet are not exactly in front of his feet, but offset like this:
0.....0
....0.....0
5. He might. But the question is, do you want it or not? If you don't mind - then let it happen. If you don't want - just take his hands off your hips, or turn in such a way that he'd have take them away.
