Inform the her boss and the president or whomever you have reported them to the IRS for withholding a W2 via a certified letter. copy them on the letter submitted to the IRS. Companies are required to provide W2s by a certain time. She is probably waiting for the last day to send it if that has not passed.

An employer has until Jan 31st to have w-2's postmarked. To allow for mail delivery, IRS allows for 14 days. If you have not received this information from your former employer by then, call the IRS as the former employer would have filed his own employees w-2's to them by that date. They should be able to give you all the same information on the w-2. Or you could do like the other answer gave, send out a certified letter and keep a copy for your records and submit to IRS upon filing your return. You many have to amend your return when you get this information if you have not received it by the time you file your taxes.

It's against the law for a company not to send you your W2 form. There's many reason why you wouldn't get it i.e., address change but company's have until Jan 31st to get to employees - you can contact the IRS or the Corporate office of your employer.
If you still do not receive your W-2 by February 14th, contact the IRS for assistance at 800-829-1040. When you call, have the following information:
Employer's name, address, city, and state, including zip code.
Your name, address, city and state, including zip code, and Social Security number

Send the company a certified letter with a "return reciept"
request and request the w-2.
Send the letter to this womans boss and copy the US Treasury Dept.
You will get it.

By law they have untill the end of January so they are already breaking the law. I would call a tax place and just ask them what they think. They will usually give you advice for free. But make it a reliable place like HR Block or etc.

If she doesn't send it when you request it, talk to her supervisor. Say you requested it but didn't receive it.
The company is required by law to provide it.

contact your HR dept. if you had one, if not, contact your old boss directly and explain the situation. If that doesnt work, go into your old workplace and demand them from someone else
