Leonardo DiCaprio did an incredible performance on The Basketball Diaries. Based on Jim Carroll, true story of a high school boy who had all the opportunity in the world at his feet, and blew it all.
River Phoenix did his best with My Private Idaho, but it's not one of my faves. A little out there.
Homegrown has an all star cast including Jamie Lee Curtis and Jon Bon Jovi.

Other than that I would have to say Trainspotting(heroin) or The Acid House, both movies based on novels by Irvine Welsh and his Ecstasy (begins shooting in March of 2007)
