It seems that they are mixed up anyway. What's going on?

They are only mixed up if people choose to login at the USA Yahoo page rather than the page for their specific country. When you choose "All English questions", you may see questions from people who login at http://au.answers.yahoo.com/ (Australia or New Zealand), http://uk.answers.yahoo.com/ (United Kingdom or Ireland) and elsewhere. If you choose "USA questions only", you only see questions from people who login at http://answers.yahoo.com/

Maybe you mean to say you sometimes see questions or answers from a Canadian who has logged in at http://answers.yahoo.com/. Yes, that can happen. But most people in U.K. or downunder will only use their local Yahoo page because they want news and shopping pages for their part of the world.

English dictionaries come in two different types, British and American. The spelling is different such as theatre (British) and theater (American).
