why start this baby out on formula? even if you don't nurse past the first few months, the baby will still get important nutrients and immunities from you as long as you breastfeed. And if you "dried up" at 5 months, was it because you were supplementing with formula? That will certainly diminish your supply.

The doctors recommend a daily intake of 5oz. per kilo of your baby's weight, or, translating to pounds, 2.27 oz. per pound.
That is: baby's weight, 3 kg. (average weight at birth)
Calculate as follows: 3kg * 5 oz = 15 oz.
Your baby will be drinking about 15 ounces a day, maybe in 7 bottles with 2.5 ounces.
With my first daughter I only breastfed... the whole first year. With my second I breastfed and used a medela breast pump to collect the milk and give it to her in the bottle while at daycare... until 10 months.
Talk to your pediatrician, he knows best.

depends on the baby my son he had any where from 5 to 11 bottles a day and was 8 oz bottles and all so was eating really good and all now he is one and doing great

quite often the amount will increase with age, if not consault a pediatrician immediatly. otherwise there are many information websites out there. enfamil.com has loads of information including signup specials like free formula and coupons and free diaper bags etc. and groups like la leche (laleche.com) have a lot of info as well, or simply surf the web. also if you want to check out freestuffformom.com it hs a lot of free stuff and coupons! good luck and congrats!
