Cost will vary from shop to shop and based on person who does the piercing. Everyone charges something different.
Keep in my the material of the jewelry can effect price as well, with titanium and niobium costing more than stainless steel.
Expect to pay anywhere from $30-$65 bucks.

Depends on where you go. Call around for different prices. I charge $10 per piercing at my shop and the jewlrey ranges between $10-30. I know other places that charge up to $40.
