There are two different types of systems.
The most common is one that the handbrake cble actually applies a force on the rear of the rear wheel calipers. This will force the calipers into a locked position.
There are quite a few cars that actually have a small drum brake setup in the center of the rear discs. These never wear out, or need service because of infrequent use. If of course there is a malfunction, or you forget to release the park brake it may wear out those shoes. This system is the better of the two because of the redundancy (like two or three seperate control systems on aircraft). It is costlier however so not used much.

One type uses a parking brake drum inside the rotor, called a disc/drum assembly, and is a smaller version of a drum brake. Or, instead of using an auxilary drum and shoes, rear discs have a mechanism that forces the pads against the rotor mechanically. One method is ball-and-ramp arrangement, another has a threaded, spring-loaded pushrod. As the brakes are applied, a mechanism rotates or unscrews the pushrod, which in turn pushes the piston out. Other types include the use of a variety of cams.

You will find that the rear discs's have a deeper hub because inside are a small set of handbrake shoes.
These work in the same way as regular shoes on cars with drums.
Or the rear brake caliper will have a cable going to it that once pulled will lock the rear brakes on.

Either by mechanically forcing the rear caliper to clamp, or by a small set of brake shoes inside the rear disc.

The handbrake cable pulls on levers that press the pads onto the disks at the rear of the vehicle. The first job I had on my first car (Cortina MkII) was to free these levers up which were corroded.
