baby outfit, diapers, rattles, any handheld baby activity toy, teething toy, as a 5 month old will need something related to their age. If the gift is to be given now at 5 months old, find something for her to 'grow' with. Something she will enjoy even into 12 months. Handheld rattles, or small activity toys with sounds always delight a baby. A small crib toy that baby can view when laying down. Nothing smaller than the tube from toilet paper, if a toy fits thru that tube, a baby can fit that item in its mouth and choke.

The parents will appreciate either clothing or a savings bond.
If you are close with the family, you could give a cute jewelry box and/or a pair of pierced earrings - as a keepsake for the future.
Most babies have more than enough toys and blankets, and unless the parents have a huge house, they will probably not want any more toys laying around.

Some books. Perhaps ones that you particularly liked when you were little, then you could write a little personal note on the inside of the cover. I have quite a lot of books for my 6 month old, but would always welcome more because other people open your eyes to wonderful books you never knew were out there.

I would suggest a bright and colorful toy. It could be just a set of plastic keys. Babies love simple things and its also affordable.
