Each area/department had one or two people (managers) stand watch over everyone in that one area. Noone was permitted to leave said area without checking with the manager, if they didn't then they were fired, no warnings. No breaks of any kind (including bathroom) without permission AND someone to fill in for that few minutes. This was common for factories like auto assembly plants, textiles, etc.
In office settings you still had management, but there was also the manager's appointed "office pet" who reported anyone who left without permission or took longer than they should for breaks. Noone knew who the "office pet" was except management.
