Is it Fear of Faliure? Fear of maintaining success once you feel you've reached it? Fear of not being "good enough"? Or feeling that the dream is larger than ones self? Enlighten me.

What holds us back is if we look externally for the answers.

If we are clear with ourselves what we want we will persue it even if someone tells us that it is not possible.

I have found if you doubt yourself, the world will doubt you too.

If you stay confident in your vision by visualising it as a reality you will be telling the world who you are and where you are going.

Overthinking the decision on going after your dream. Once you've made enough excuses of why not to pursue it, the dream has lost it's luster, which proves it's only a dream in passing and should not be fully weighted on whether you are an underachiever or not good enough.
"Just do it" is a fantastic mentality, but you will need to decide what is an acceptible level of what you have done so far has accomplished the goal you wanted to achieve. Then you move on. Most important is to try and execute your actions w/ every bit of passion you have in you or else it'll only be half-assed.

Usually it's a lack of discipline and planning. Often both of these come from a feeling of inadequacy. We fail to discipline ourselves, because we fear failing to follow through on our disciplines. We fail to plan because for the same reasons.
