OK, so one of my sims has this friend Don who is sick with food poisoning. He comes on a visit from time to time and always pukes in the toilet, leaving it dirty and this lasted very long already. Is there some way to deal with him?

I've never had a sick friend on the game but I've had a sick sim and this is what I did. If you click on your sim on the computer click shift and then c at the same time a cheat box will come up and all you have to do is type UnlockCareerRewards in the cheat box. Then go to the rewards symbol while you're still on your sims. The career rewards should be unlocked then.Next you can get the medicine maker thing and make medicine and maybe try to give it to them.

The last time I had a sick friend I stop inviting him over. He would be sick and go to the toilet on my floor all the time. I stopped being friends with him.
Sounds mean, but my sims were more valuable then he was.
