First answer is when the grip looks either used or dirty (amazing how ugly a grip can get after sweating on it for a while).
It is a good practice to change it at least when you change your strings (remember that you change your strings as often in the year that you play in a week, and no less than every 6 months).
One advise: good grips can cost $6 or $7 in a tennis shop. One solution is to use overgrips. If you put a good grip on (I use one with 'contour') then use a good quality overgrip like the Wilson Pro (white or grey) on top. The overgrip is thin, so it doesn't alter your grip size, and is inexpensive enough that you can change it often (once a month?) and the grip remains untouched.

If your hand is slipping when you hit a hard ball then your grip isnt working well. Instead of replacing a grip, most people just buy an overgrip for a few dollars.

You should replace it when it starts to wear down after a while. You hand may start to slip a bit and you'll be able to tell too.

when it starts to peel off the top layers, wears out, or starts coming off.
If your grip is too slippery try to get a sweat-absorbtion grip.

Sometime you can look or don't look by feel it with your hand and when start slipping off or when you look at it coming off, its the time for you to replace it!

when your hand starts slipping off the racket it is time to replace it. if you buy overgrips it's easy and quick to do it.
