Generally the 4.0, but some districts have weighted classes that can actually get the average between 4.0 and 5.0. Just a way of rewarding students for taking harder classes.

It depends on your specific school. Ask your counselor how the GPA is calculated. Is it based on a 4.0 scale or (like my high school's case) 7.0 scale? If it's based on 4.0, then your score is pretty good...

4.0, unweighted. If you count in honors classes, etc, it's possible to get up to a 6.0 per semester in junior and senior years, 5.0 freshman and sophomore years, making overall high school GPA 5.5.

It depends. Usually schools go by a 4.0 scale (4.0 being perfect). My Highschool went by a 12.0 scale.
